- Collaborating with primary healthcare professionals and healthcare team members of experts to make a plan of treatment (Halcomb et al., 2017)
- Accessing resources like online support group, take classes to learn about it, reading online about it
- Reasons of taking care when diabetes occur
- To avoid negative outcome
Step 2: Knowing ABC OF Diabetes
- A – knowing about A1C test, outcomes, results
- B – blood pressure goal and importance
- C – knowing about cholesterol and specially about good and bad cholesterol

Step 4: Getting Routine care from primary health service to stay healthy

Present day researchers have found that obesity is intricately associated with diabetes as fat deposition results in reducing the ability of the cells to response to insulin. Cells in such conditions become resistant to insulin and therefore glucose cannot be accepted by cell causing increase in blood glucose level (Cashn et al., 2015). This conditions associates with different negative outcomes like heart disorders, stroke, increase in blood pressure, failure of organs like kidney, eyes, and nerve damage and many others. Therefore, professionals beside pharmacological management should also educate patients about different lifestyle interventions like weight management, diet, exercises, regular check-ups and many others. These would help them to be aware of their health condition and will make them serious about controlling all these factors for living healthy lives.
The handout is prepared with interesting pictures and very simple words which would help in understanding the steps that diabetic people can take to manage their health. Such easy language and the absence of medical jargons would be easier for them to relate to the interventions. The older people will also be able to read them as the handout is not clumsy but are clear and readable. A primary healthcare professional can use this to educate the patient and follow the teach-back method to understand whether they have understood it or not (DeLaune et al, 2016).
In many of the cases, it is seen that many individuals have the idea that they have diabetes but they have no ideas about how to manage them. They only follow medications but does not understand the importance of lifestyle management and time to time checkup for controlling the diabetes. They tend to follow negative habits and alongside have medications. These, do not result in positive outcome on health and diabetes remains poorly managed (Green & Gill, 2017). Therefore, such handouts act as effective resource for developing their health literacy which they can keep with them and take help from them accordingly.
