From the result of activation design, it is clear that most of the nurses and caregivers use to ask the patient about the treatment plan they are going to execute. Similarly, most of the caregivers agreed that they use to give the patient choice about their treatment plan. However, the mean score of 3.6 in the third question indicates that nurses do not usually asking patient to share their ideas regarding treatment plan. Therefore the nurses use to give the patient the choice, however their ideas are not gotten enough attention.

From the result of delivery system and practice, it is clear that most of the nurses and caregivers agreed that they are satisfied about what they are doing and organising for patient’s care. However, the mean score of 3.9 in the first question indicates that the nurses do not usually show the patients the care-giving procedure to influence their recovery speed. At the same time, they do not provide written list of things to patients so that they could improve their health themselves.

From the result of Goal setting / Tailoring, it is clear that most of the nurses and caregivers use to ask the patient about their health habit directly. Similarly, most of the caregivers agreed that they use to encourage the patients to go to a specific group or class to help them cope with their long?term condition. However, the mean score of 2.6 in the third question indicates that nurses do provide the copy of treatment plan document to patient. The mean score of 3.6 in third and fourth question indicates that the nurses use to help the patients to set specific daily basis goals for taking care of themselves and the nurses also talk to patients regarding their goals.

From the result of Problem solving / contextual, it is clear that most of the nurses and caregivers use to ask the patient about how the long-term condition affects their life. However as per the score of 3.3, it can be said that sometimes the nurses help the patient to plan for taking care of themselves. Similarly, in not every day schedule the nurses use to help the patients to make their own treatment plan for themselves that they can carry out in regular life. However, most of the nurses agreed that they sincerely put the values, ethics, believes of patients under consideration while developing or recommending the treatments.
